We are now able to accept referrals for one-off consultations with parents and carers, and for both professional referrals and self referrals for ongoing therapy.

If you need urgent help, please use the resources here.

If you are a professional hoping to refer a child or family, please email info@appletreecentre.co.uk and we will send you the appropriate referral form. We are also happy to discuss any potential referrals on the phone to help you decide whether or not we are the most appropriate service.

We are now able to accept self referrals for regular therapy appointments and one-off parent consultations. If you are paying directly for therapy for a family member, all routine appointments are currently charged at the flat rate of £70. One-off supportive consultations are charged at £105, including a slightly longer (up to 60 minute) first appointment and a brief written summary.

Please fill out the following form, giving as much information as you can, to be placed on our waiting list. All questions must be answered before the form can be submitted. Please do make sure your form has been submitted successfully before leaving this page - you may need to scroll down slightly to see the confirmation or any error message.

We will respond to most enquiries via email. Please check your spam filter if you have not received a reply within 3 days.

    Your name*

    Email address*

    Phone number*

Email addresses of any other parents/carers who should be copied into correspondence at this stage. (This form will only be sent automatically to the address above, but we will reply to all addresses listed here. Please let us know if you are including any information that you would prefer us not to share with other parents at this stage.).

Child/young person's date of birth (up to age 25)*

It is generally best practice for us to contact all parents before beginning direct work with a child unless there's a good reason not to do so, and we may not be able to offer therapy without this. Do you expect that all parents of your child will consent to them receiving therapy?*

Brief description of the reason for your referral*

Do you live in or near Sheffield?*

What is the first part of your postcode?*

Is your child adopted or Looked After (subject to an SGO or other care order)?*

    Have you contacted us before?*

Is there anything else we need to know, to make sure that we offer the most appropriate form of therapy for your child?
    Please include, for example:
    - any other professionals currently working with your child or family (eg social worker, other mental health or support worker)
    - any personal relationships with therapists working at the Apple Tree Centre, or any other reason why one of our therapists would not be suitable
    - if you are intending to seek funding for therapy by any means other than direct personal payments
    - any neurodiversity or learning difficulty
    - any communication or other access needs

We offer regular therapy appointments on week days, Monday-Friday, starting between 9:30am and 5:15pm. We have very few appointments available from 5:15 onwards and we do not recommend appointments after school for very young children. We are only able to offer a limited number of options to each family so please do let us know if your availability changes.

The wait for after school appointments is much longer than appointments during the day. We can provide a letter to schools to support children leaving to attend sessions at the Apple Tree Centre.

Would you like to be offered regular online therapy sessions for your child?*

Would you like to be offered regular face-to-face therapy sessions for your child?*

Would you accept therapy appointments during school time? (This is likely to reduce waiting times significantly.)*

Please list all the days and times you would accept a regular weekly therapy appointment. *

Would you like to be offered regular Therapeutic Parenting Support and/or Family Therapy appointments?
These would be appointments for you as parents/carers, or for your family as a group if your children are old enough to engage with online sessions in this way. We currently offer online Family Therapy on Tuesdays and Wednesdays.

If you are looking for Therapeutic Parenting Support / Family Therapy, please list all the days and times when you would accept weekly or fortnightly appointments. (If you do not want this form of support, please type 'na'.)*

Would you like to be offered a one-off parent/carer consultation appointment?
This would be a supportive consultation for you as a parent/carer with one of our experienced therapists, to explore your child's needs and offer advice and guidance about how you can best support them. We would ask you to come to this appointment without your child.
This will not affect your child's place on our waiting list and it is likely that the therapist who offers you a one-off consultation will not be the same person as you child eventually sees for regular appointments.*

If you are looking for a one-off consultation, please list the days and times when you are available to attend. (If you do not want a one-off consultation, please type 'na'.)*

When you reach the top of our waiting list we will get in touch with you via email and offer you a small number of appointment options. If you are paying directly for therapy for a family member, all routine appointments are currently charged at the flat rate of £70. One-off consultations are charged at £105 per appointment, including a brief written summary.

We are currently expecting significant waiting times, depending on your needs and availability. Please confirm you would like to be placed on our waiting list.*

A confirmation message will appear at the bottom of the screen after you click 'send' and a summary will automatically be sent to your email address. If you do not receive an email within the next few minutes, please check your spam/junk folder and mark our email "not spam".

If you need urgent help, please use the resources here.

If you need to contact us urgently, the quickest way is usually by email to info@appletreecentre.co.uk.

We endeavour to respond to all messages as quickly as we can. We respond to the majority of enquiries within 3 working days, so if you have not had a response by this time, please check your email spam filter or follow up to ensure that we have received your message.
